Where you can go, and what each place is like
We have two New York locations, and Fidelis applies to both. Our Florida clinic in Aventura doesn’t participate in Fidelis, so if you’re a Fidelis member, the two New York addresses below are your spots.
Manhattan (Midtown)
Our flagship is at 515 Madison Avenue, 21st Floor, at the corner of 53rd Street and Madison. Take the E or M train to 5 Av–53 St and you’re steps from the entrance, or the 6 to 51 St and walk a few minutes north. We run all four in-clinic treatments here: Spravato (esketamine, an FDA-approved nasal spray), transcranial magnetic stimulation (TMS), IV ketamine, and medication management.
Plan for a few hours, not a quick appointment. A Spravato visit includes about two hours of monitoring after your dose, so this isn’t a lunch-break errand. Bring something to listen to, and arrange a ride or plan to rest before you head home.
Rockland County (New City)
Our Rockland clinic is at 200 E. Eckerson Road, Suite 280, New City, NY 10956, just off NY-59 and minutes from the Palisades Interstate Parkway. Phone: (845) 682-7200. “Fidelis” isn’t one thing, and which one you have changes everything
This is the part almost no one explains. Fidelis Care isn’t a single plan. It’s a family of them, and for a medication like Spravato, the differences actually matter.
The common Fidelis lines you might carry include Medicaid Managed Care, HealthierLife (Fidelis’s HARP, or Health and Recovery Plan, for adults with significant behavioral health needs), the Essential Plan, the Marketplace metal-level plans (the bronze, silver, gold and platinum Qualified Health Plans you buy on the state exchange), and Wellcare By Fidelis Care Medicare and Dual Advantage plans.
Here’s the piece that trips people up. A treatment can be paid for two different ways: through your medical benefit, which is the side of your plan that covers office visits and procedures, or through your pharmacy benefit, which is the side that covers the drug itself. Think of it like a restaurant that bills you separately for the table and for the food. For some Fidelis lines, the esketamine and the visit are handled the same way; for others, they’re split. For example, Fidelis routes pharmacy benefits for its Medicaid Managed Care and HealthierLife (HARP) members through NYRx, New York’s Medicaid pharmacy program, while other lines use a different pharmacy setup. You don’t need to memorize any of this. You just need to know it exists, so the answer you get on the phone makes sense.

The certification thing nobody explains
There’s a federal safety program behind Spravato called REMS, short for Risk Evaluation and Mitigation Strategy. In plain terms: because esketamine can cause short-term dissociation and a rise in blood pressure, the U.S. Food and Drug Administration (FDA) requires that it be given only at certified sites. You take the dose there, under supervision. You don’t take it home. Someone monitors you for about two hours afterward, and you arrange not to drive yourself until the next day. Dissociation here means a temporary, spacey, floaty feeling that usually passes within the monitoring window. Both of our New York locations are REMS-certified.
Here’s what the insurance conversation usually misses. A clinic can accept your exact Fidelis plan for therapy or psychiatry and still not be able to give you Spravato, because it isn’t REMS-certified for esketamine. So “do you take my insurance?” is the wrong first question. The better one is: “Are you REMS-certified for Spravato, and do you take my specific Fidelis plan?” Ask both, together. Fidelis Care Spravato coverage: the paperwork, and who does it
Fidelis, like most insurers, wants prior authorization before it will approve Spravato. In practice that usually means documentation of your diagnosis and a record of antidepressants you’ve already tried without enough relief. Our team prepares and submits that paperwork for you, so you’re not the one on hold with the insurer. If the first answer is no, that’s often not the end. There’s an appeals path, and we’ve been through it enough times to know what works. We won’t promise an approval, a timeline, or a number, because those genuinely depend on your plan and your history.
